The HAP Foundation invites Community Health Workers (CHWs), public health strategists, health care leaders, and community activists to join us for a day of learning, networking, and celebration of the achievements of CHWs across the region.
The all-day summit will be held on January 24, 2024, from 8:30 am to 5 pm, at 7059 S. South Shore Dr. Chicago, IL 60649.
